Pension/Retirement Forms

Dear Pensioner/Retiree:

In order for you to receive your pension/retirement benefits, you'd need to renew your forms every year through the KRG Representation based in Washington, if you live in USA, Canada, or Mexico. 

The following forms are needed to fill out and mailed to the KRG Representation for processing. Once processed, they will be sent back to you to then send it to Kurdistan:

1. Fill out the Pension/Retirement: Download and save it to your desktop. Then open it in Adobe Acrobat Reader as a standalone file and not in any browser:

-Complete two copies of the standard Kurdish form.
-Include a copy of the applicant’s US/Canadian identity card and an Iraqi ID card.

-Note: No English form or notary public is required for this form. Please refer to the second page for an English explanation. If you have the ID requested under item #13 (National ID/Nistimany Card), you can skip items number 10, 11, 12, 14, and 15 on the form.

2. Fill out the Special Power of Attorney: Download and save it to your desktop. Then open it in Adobe Acrobat Reader as a standalone file and not in any browser:

-Complete two copies of the standard Kurdish form.
-Complete one copy of the English form in the presence of a notary public.
-Include two colored passport-size photos of the applicant.
-Include a copy of the applicant’s US/Canadian identity card and an Iraqi ID card

3. Fill out the Certificate of Life: Download and save it to your desktop. Then open it in Adobe Acrobat Reader as a standalone file and not in any browser:

-Complete two copies of the standard Kurdish form.
-Complete one copy of the English form in the presence of a notary public.
-Include two colored passport-size photos of the applicant.
-Include a copy of the applicant’s US/Canadian identity card and an Iraqi ID card

Once processed, documents will be returned by regular postal mail. To expedite the return of the documents, include a pre-paid self-addressed return mailing envelope (USPS ‘Express Mail' or FedEx). 

This service is free of charge.

The application process time is three weeks (15 business days), once all documentation is received at the KRG-US office.
