Certificate of Life

The KRG Representation in the US is able to provide proof of life documentation, often required to receive retirement and other government benefits.

  • Complete three copies of the standard Kurdish form.
  • Complete one copy of the English form in the presence of a notary public.
  • Include three colored passport-size photos of the applicant.
  • Include a copy of the applicant’s American identity card and Iraqi ID card (Jensya).
  • Include the applicant’s contact information: full name, address, phone number, and email (if available).
  • Mail all of the above to:

Once processed, documents will be returned by regular postal mail. To expedite the return of the documents, include a pre-paid self-addressed return mailing envelope (USPS ‘Express Mail’ or FedEx).


Application process time is 15 business days (3 weeks) once all documentation is received in the KRG-US office.
