Certificate of Life


The KRG Representation in the US is able to provide proof of life documentation, often required to receive retirement and other government benefits.

 

 

Instructions:

  1. Complete two copies of the standard Kurdish form.
  2. Complete one copy of the English form in the presence of a notary public.
  3. For retirement/pension only: complete two copies of the pension form ---> 
  4. Include two colored passport-size photos of the applicant.
  5. Include a copy of the applicant’s American identity card and Iraqi ID card (Jensya).
  6. Include the applicant’s contact information: full name, address, phone number, and email (if available).
  7. Mail all of the above to:

The application process time is 3 weeks (15 business days), once all documentation is received at the KRG-US office. Please include a pre-paid, self-addressed FedEx or USPS envelope.
